Scheduling Your First Appointment

What to Expect at Your First Visit

At the time of your first appointment, you will be requested to complete a brief health history and a patient registration form.  We will mail these forms to you in advance whenever possible, or you may click on the bolded words below to download and print your own Registration and Medical History to bring with you to your appointment.

Transferring Records From Another Office

If you are transferring to us from another office, please contact them and have your x-rays and records forwarded to our office.  This may well minimize the need for additional X-rays and keep your costs to a minimum.

What to Bring for Your First Visit

If you have dental insurance, please remember to bring the necessary information with you. Your initial visit is very important and will include a dental exam, periodontal evaluation, oral cancer screening, and a consultation concerning your dental needs.
